What is Chronic Disease Management (formerly Enhanced Primary Care or EPC)

A Chronic Disease Management (CDM) Program is a means for General Practitioners (GPs) to assist the management of the health and well-being of patients with chronic or terminal medical conditions (a medical condition that has been or will be present for a minimum of 6 months). This means patients may be eligible for Medicare rebates for certain Allied Health Services through the GPs referral.

How do I get a CDM Program referral?

The CDM program will be provided by your GP if you are deemed eligible for this service following a clinical judgement from the GP after considering a patient’s medical condition and healthcare needs. This program allows for a maximum of 5 Allied Health Service rebates under Medicare per year.

What do I do once I receive the referral?

Once you have received your CDM referral from your GP, you can contact our clinic to make your appointment. You will need to mention you have a CDM plan when making your appointment so the appropriate coding can be used.
